Parents - Registration

Important News Regarding Meal Assistance: The application for meal assistance for 2015-16 is now available under the Nutrition tab located on the left. If your family receives Food Assistance, you may be directly certified and may not need to complete an application. We will be notifying you if we received your child’s name automatically from DHS within the next 10 days. Please remember if you receive a letter of notification about Direct Certification from the Nutrition Department you do not need to fill out an income application, but you must fill out the Fee Waiver Form to qualify for other school fee reductions. The fee waiver form is also available under the Nutrition tab for your convenience. Only signed fee waivers received before September 30th will be honored. If you have any questions, or need help filling out the application or fee waiver, please contact the Nutrition Department at 515-981-9876. Thank You!

Registration Forms for Returning Students 2015-2016 PDF  | Print |
Parents - Registration

Attached are the 2015-2016 registration forms for returning students. Hard copies will not be mailed this year. Please print the forms and mail them to the building. You may also bring them to the building starting August 5th or bring them to one of the walk-in registration nights at Eastview 8/9 cafeteria on August 6th and 11th from 4-7 p.m. If you choose to register in person, please make sure you print the forms and bring them with you. Supply lists and transportation request forms may be found on the left-hand side of the page under Supply Lists and Transportation. They are also located under Parents at the top of the page. These forms are for RETURNING students only. If you are new to the district, please contact the registrar, Cheri Atherton, at 515-981-0676 to schedule an appointment to enroll your student.

Change In Pay-for-Ride Status for 2nd, 5th, and 8th Graders PDF  | Print |
Parents - Transportation
As 2nd graders and 5th graders move to a new attendance center, their transportation status may change from pay-for-ride to free or vice versa. The same holds true for 8th graders as the pay-for-ride area changes from 2 to 3 miles when they start 9th grade. For more information, and to help determine which form to complete for next year's transportation, please read the attached letter.
